1. Each student MUST print his/her attendance sheet which will return together with filled evaluation sheet either at the and of the course or before the examination.
Always come to a clinical lesson with white gown, some shoes to change and Medical Faculty badge. One missed signature is accepted only to be credited.
Before enrollment to the course, the students are obliged to print out following forms. The forms can be downloaded here and here.
Terms for the exam will be available on the SIS until the end of the course. Only one missing teatechrs signature is accepted to be credited.
2. Students are kindly asked to come and change themselves in advance enough to be ad the departments on time!
StomSurg
Come to The Department of Maxilo-Facial Surgery, building A11, ground floor, 8:00 a.m. Before coming to the Stom Surg pick up a locker room key from a secretary of KARIM (A6, the "white arrow" entrance), change to white gowns, return the key and then go to the Stom Surg. A contact person: I. Darázs, phone: 702 192 385. In his absence, go to the operating room, you will be taken in by the deputy teacher.
Urol - ICU
Come to The Department of Urology (Ke Karlovu 6), ICU, 1st floor at 8:30. First, take a locker room key at the ground floor (Medical Card Index Office) and change yourself in the basement. A contact person: Y. Blazkova, phone: 608 770 094.
OG - OR
Starts at 8:00 a.m. Take the left stairs to the second floor (A "Tower") and knock the door signed KARIM. No gowns are needed! Take shoes to change with you, pls. A contact phone number: 22496 7241.
ICU1, ICU2
Come to The Departement of Cardiovascular Surgery, building A2, 2nd floor at 9:30. A contact person: Pořízka, phone 702 089 475. No gowns are needed. A locker room accessible with an ISIC card is in the groud floor! Please before going to the ICUs leave there your personal belongings.
ICU3
Come to The Department of Anesthesiology and Intensive Care, building A6, ground floor at 9:30, ER "back" entrance (the same floor as the lecture hall, other side of the corridor). No gowns are needed! A contact person: Otáhal, phone number: 724 120 970.
Surg1 ORs
Mon starts at 8:00 a.m., Tue - Fri starts at 7:30 a.m. Building A8, 2nd floor. Before going to the Surg1 pick up a locker room key from a secretary of KARIM (A6, the "white arrow" entrance), change to white gowns, return the key and then go to the Surg1 ORs. CS - OR
Mo starts at 7:50 a.m. Tue - Fri starts at 7:00 a.m. Go to the building A2. Change in a locker room on the ground floor. The door opens using an ISIC card. After changing go to the OR. No gowns are needed! A contact person: Pořízka, phone 702 089 475.
